BY SIGNING BELOW: The employer (“Group”) confirms acceptance of the benefits, rates, terms, provisions, and disclosures outlined in this proposal and understands that the medical plans are not considered creditable coverage under ACA. Group agrees that they are solely responsible for any obligations under ACA, ERISA, and any other applicable insurance and benefits laws or regulations, and that Coterie and the insurance carriers assume no liability. It signifies the Group’s request to proceed with the insurance application for the carrier to issue the insurance policy to the Group in the company’s name and/or to Unified Employers Trust on behalf of the Group. Group agrees to sponsor the employee benefit offering through endorsement and promotion to their eligible employees with internal employee engagement activities and through permitted employee engagement activities via eligibility data file directly from the app. Acceptance of this proposal shall initiate the implementation process.
